WAIVER OF RIGHTS AND CLAIMS

BY VOLUNTEER PARTICIPANTS

The undersigned individual intends to participate as a volunteer in repair projects sponsored by Rebuilding Together San Diego, a California non profit corporation, and will receive no compensation or remuneration for services.

CALIFORNIA LABOR CODE 3352(i) EXCLUDES PERSONS PERFORMING VOLUNTARY SERVICES FOR NON PROFIT ORGANIZATIONS FROM REQUIREMENTS FOR COVERAGE BY WORKERS COMPENSATION INSURANCE.

THE BOARD OF DIRECTORS OF REBUILDING TOGETHER SAN DIEGO DOES NOT PROVIDE WORKERS COMPENSATION INSURANCE COVERAGE FOR VOLUNTEER PARTICIPANTS.

THEREFORE, in recognition of the above, and in consideration of the opportunity afforded to participate in Rebuilding Together San Diego projects, the undersigned hereby waives any and all rights to any and all claims, causes of action, demands, or damages of any kind or nature whatsoever for liability for injury to my property or person, or the property or persons of my employees or dependents, whether known or unknown, foreseen or unforeseen, existing, claimed to exist, or which can ever hereafter arise against Rebuilding Together San Diego, its Directors, officers, employees, successors, owners of the project site or assigns. Authorization: I consent to the unrestricted use by RTSD and/or any person authorized by them of any photographs, recordings, interview, videotapes, motion pictures or similar visual or auditory recording of me created in connection with the Project.

I have read and understand the foregoing waiver of rights and claims and agree to abide by the terms and conditions thereof.

Parent Or Guardian Authorization and Release for Minors:

I am the parent or guardian of the above referenced volunteer (who is 16 or over). I authorize his or her participation in the identified project. On my behalf and on his or her behalf, I hereby release and waive any and all rights to any and all claims against, Rebuilding Together San Diego, its directors, officers and employees collectively and individually from causes of action, cost or expense arising as a result of his or her participation in said project, including, without limiting the generality of the foregoing, those resulting from personal injury to he or she , or damage to his or her property sustained in connections with the activiteis in the project renovation project.
